Snakebite Envenomation in Colombia: A Priority Challenge for Public Health

1. What is Snakebite Envenomation?

It is defined as the set of signs and symptoms in the body caused by the inoculation of toxins through the bite of a venomous snake. Envenomation can produce local effects (such as necrosis, severe pain, and edema) and systemic manifestations (coagulation disorders, hemorrhage, renal failure, or neuromuscular paralysis), depending on the snake species involved.

In Colombia, these events are mainly classified according to the reptile family involved:

  • Bothropic, crotalic, and lachesic envenomation: Caused by snakes of the Viperidae family (mapaná, rattlesnake, bushmaster). These are the most common and are characterized by their hemotoxic, cytotoxic, and myotoxic pathophysiological activity.
  • Elapid envenomation: Caused by snakes of the Elapidae family (coral snakes). 2. History of the Event in the Public Health Context

In Colombia, snakebite envenomation has been under surveillance since 2004, when the National Institute of Health (INS) incorporated it into the Public Health Surveillance System (Sivigila). This mandatory inclusion enabled the systematic collection of data, facilitating case georeferencing, the identification of vulnerable populations, and the strategic distribution of antivenoms.

3. Epidemiology in Colombia

Due to its geographic location and diverse altitudinal zones, Colombia is home to one of the greatest diversities of snakes in the world. The epidemiology of this event reflects a clear sociodemographic and environmental distribution:

  • Incidence: Thousands of cases are reported annually across the national territory. The regions with the highest notification rates are usually the Orinoquía, the Amazon, the Pacific region, and the Middle Magdalena area.
  • Affected population profile: This event predominantly affects males of working age (between 15 and 49 years old), engaged in agricultural activities (coffee, palm, and cacao growers, as well as harvest workers), in addition to Indigenous and Afro-descendant communities living in dispersed rural areas.
  • Predominant species: The genus Bothrops (particularly Bothrops asper, commonly known as “mapaná x”) is responsible for more than 70% to 80% of snakebite envenomation cases in the country.

Prevention of snakebite envenomation is based on community education and environmental management, especially in highly endemic areas:

  • Use of physical barriers: Wear high rubber or leather boots and thick pants during fieldwork or while walking along cleared trails.
  • Extension tools: Use sticks or poles to inspect the ground before placing hands into crops, leaf litter, or accumulated vegetation.
  • Household environment control: Keep the surroundings of homes clean and free of debris, stacked firewood, and waste that may attract rodents (the primary food source for snakes).
  • First Aid Training: In the event of a snakebite, the primary measure is immediate transfer to a healthcare facility. The use of tourniquets, incisions, venom suction, or the application of homemade substances should be avoided.

5. Conclusion

Snakebite envenomation in Colombia extends beyond the medical sphere and has become an indicator of socioeconomic and environmental vulnerability in rural settings. The key to mitigating its impact lies in the continuous strengthening of epidemiological surveillance through SIVIGILA, ensuring timely data flow to optimize the availability of polyvalent antivenoms in the most remote areas. Reducing morbidity and mortality from this cause requires maintaining strong coordination between community-based preventive education and the response capacity of local health services
